ACH Payment Authorization

By submitting your bank account information, you authorize Cooks Bay LLC dba Plaid Moose Creative to initiate ACH debits from the account provided for payment of all amounts due under the applicable Master Services Agreement (“MSA”) and any related Scope(s) of Work, including invoices and recurring fees, if applicable. You represent that you are authorized to approve transactions for this account and that the information provided is accurate.

This authorization will remain in effect until revoked in writing. Revocation does not relieve you of payment obligations under the MSA or any Scope of Work. All ACH transactions are processed in accordance with applicable NACHA rules and governing law.
